The Centers for Medicare & Medicaid Services (CMS) Ratings System
The Centers for Medicare & Medicaid Services (CMS), a federal agency within the United States Department of Health and Human Services, has created a five star quality rating system for nursing homes. Facilities are reviewed on a 1 to 5 star rating scale with a 1 indicating below average and a 5 indicating above average quality. These ratings for nursing homes across the United States can be found on the federal government website at Care Compare on Medicare.gov.
How the CMS Rates Facilities
There is one overall rating for each nursing home and a separate rating for each of the following:
- Health Inspections. This includes the three most recent inspections and investigations due to complaints. The inspection team investigating complaints, facility reported incidents, and any deficiencies cited will determine whether a nursing home has met the minimum quality requirements from Medicaid and Medicare to ensure resident life safety code is adequate. The most recent findings are weighted over prior years.
- Staffing. This rating includes information about nursing and administrative staff and the average number of hours of care that are provided to each nursing home resident. This rating takes into account different levels of care provided by a home. Nursing homes providing care for individuals with more severe needs would be expected to have more nursing staff to ensure adequate resident health.
- Quality Measures. The quality measure (QMs) includes information on how well nursing homes are caring for residents’ physical and clinical needs. This is based on 15 different physical and clinical measures.

New York State Department of Health Nursing Home Profiles
The New York State Department of Health also provides nursing home ratings intended to allow patients and their families on Long Island and throughout New York to easily determine the performance of NY nursing homes. They include an overall rating as well as ratings in five domains of care including:
- Preventative care

- Quality of Life
- Resident Safety
- Resident Status
